Core of Knowledge: Physical Needs - Understanding Individual Program Plan (November 2025)
Presenters will discuss the procedures and timelines for Individual Service Plan ( ISP), Best Practices to keep in mind when creating an ISP, Important elements of Person-Centered Planning approach, language to use when writing the ISP, and Components of an ISP. Looking at ISP quality checklist and guidelines for what makes a good Individual Service Plan.
By the end of the course, participants will:
1. Demonstrate knowledge on how to create an ISP
2. Demonstrate knowledge on applying best practices standards to real life situations.
3. Demonstrate knowledge of re-writing goal statement using person-friendly language.
4. Demonstrate knowledge of writing measurable objectives.
5. Demonstrate knowledge of writing objectives and plans.
